Another frequent mistake is the "urgent" trick.
It's likely you've been tempted to try something like this yourself.
"John, this is an emergency. Really. You have to call me immediately."
The person getting such a call probably won't respond. It won't go well if they do. They'll find out that the only urgent thing was your desire to speak with them. Then they'll be annoyed and will be even less inclined to respond to you another time.
Now things are worse than they were before.
Those are the two biggest mistakes most of people make when they leave a message for their ex.
